Fill out your name, address, solving categories, and set(s) of puzzles you wish to purchase and mail this form, with a check or money order (U.S. funds only, please) to American Crossword Puzzle Tournament, 55 Great Oak Lane, Pleasantville, NY 10570.

We'll send you a full set of the tournament puzzles. Solve them, and return the solutions to us with your times. We'll judge your puzzles, send you your scores, and tell you your ranking (against the actual contestants at the tournament) in every event in which you were eligible to compete!
